On Sunday, I was lazy. I couldn't walk on the treadmill. I slept in the morning and had a short afternoon nap. As a result, I could only do 7910 steps on Sunday, according to the Fitbit App!
At work, I consulted 28 patients in my chamber. Hence, it was neither hectic nor too relaxing! There was no emergency case. As it was a Government holiday, only a handful of people came for the visa medical check-up. I examined a few of them.
The suspected renal stone case I talked about in my previous post, that patient returned on Sunday to show me the Ultrasound scan report. The report revealed bilateral renal stones with an impacted stone at the lower end of the right ureter. That impacted stone was causing severe pain. I explained to him the report and added a few more medicines.
Another tense expatriate came to show his sugar test report that he had done on his own behalf. For the last couple of days, he was peeing more. The sugar level at the fasting state was quite high (249 mg/dl). He was shocked to see his sugar level. According to him, around 5-6 months before, he did an HbA1c test, one of the best tests for diabetes. It was within normal range. But, this time, it showed 11.65%, pretty high!! He was reluctant to start sugar medicine, but he had out of options!

Sundowning
Sometimes, people with Alzheimer’s or other forms of dementia manifest symptoms like agitation, confusion, or mood swings in the late afternoon or at early night (when the sun goes down). This phenomenon is termed sundowning!
In the early stage of Alzheimer’s, the symptoms may not be apparent. However, as the disease advances, sundowning becomes more obvious slowly but surely! Almost all Alzheimer’s patients will have sundowning to some extent! A person with a sundowning phenomenon may have a good day (fewer symptoms) or a bad day (more symptoms)!
Changes in the familiar surroundings (e.g., events where many people gather in the patient’s room, noise), changes in the daily routines, physical illness (pain, infection, constipation, lack of sleep), etc, are the triggering factors for sundowning symptoms.
