I love spicy food. If I'm not sweating, it's not hot enough. Most people, however, are not like that. They watch me dump a whole pack of crushed red peppers on a single slice of pizza and look at me like I'm nuts. Most don't appreciate heat like I do.
My wife is one of these people. In fact, she's more sensitive to spicy foods than most. Most of the time it's not an issue and she knows what she can handle and what she should avoid.
I suggested ordering food from a nearby restaurant called Matador, which specializes in more "upper scale" Americanized "Mexican" food. She picked out Verde Carnitas. It seemed mild enough. There was no mention of peppers or anything remotely spicy.

Wrong! She took one bite and had to instantly spit it out because it was much too spicy for her. Fortunately, I also ordered Birria tacos, guacamole and queso dip, so she had other things to eat. I know some restaurants provide a "heat scale" on their menu items, although they're admittedly in the minority. Still, it'd be nice to see that become more common. I enjoyed the food, but it was a bummer for her, which kind of left the whole rest of the evening feeling "meh." You can't win them all, but after a rough day it wasn't a great way to end the evening.
Have you ever had a similar experience with food that was far hotter than it looked?
