Hi there. How are you? I hope you are doing great. Welcome to my short diary for Wednesday, June 25, 2025!

On Wednesday, I could walk on the treadmill for 10 minutes before lunch. At night, I couldn't add any minutes to it. However, I could do 11,061 steps on the day (according to the Fitbit app).

In the morning, I had, as usual, a second round of morning sleep. In the afternoon, I had a 20-minute power nap before joining the evening shift duty. After 9 pm, I had to make a quick trip to a cheap store to buy diapers for the newborn. My elder kid had two online classes on Wednesday. I had to assist him in both classes- one was at 2 pm and the other was at 10 pm.

At work, it was a pretty easy one with only 18 patients in my chamber. Of them, a patient had an over 12-hour cut injury on his left palm. Even though it was late, I stitched it. We found a troubling issue with him. Surprisingly, he was phobic of hand gloves!! The injections and stitching didn't bother him, but seeing me (and the nurse) wearing hand gloves put him in a distressed situation. He had a history of a bad injury to his neck. Since then, he has been phobic of hand gloves!!

A father and son came to me. They were riding a battery-driven bike the previous day. They fell from it and sustained injuries to multiple body sites. The father had more pain in his right knee. An X-ray showed no fracture. The son had a big abrasion on the front of his right knee. They were given management accordingly.

An unfortunate group of patients is those who suffer from dysmenorrhea (period pain). We have some regular ladies who come for injections for the period pain. Yesterday, such a patient came with acute dysmenorrhea, vomiting several times, extreme fatigue, limb numbness, etc- pretty sick! With injections and IV fluids, she felt somewhat better while leaving our clinic.

Effects of long-term PPI therapy [H/FB/C]

One of the most commonly used medicines in the general population is PPI (e.g., omeprazole, esomeprazole, pantoprazole, Rabeprazole, etc.), aka “gas medicine”. For any tummy upset, one of the medicines will be due to gas or acidity. Nowadays, the prazole group of drugs is preferred over antacids or H2-blockers! However, it has some demerits if used for a long time!

  • It is associated with reduced absorption of iron, B12, and magnesium.
  • It is also associated with a small but increased risk of osteoporosis and bone fractures.
  • The drugs also predispose to gut infections with bacteria such as Salmonella, Campylobacter, and possibly Clostridium difficile.
  • It increases the risk of helicobacter-associated (a bacterium in the stomach) waste away from the stomach's inner lining (medically called gastric mucosal atrophy).
